If you've come to this page, you're probably ready to get started and need the 'packet' of instructions and forms.  Great!  But, hurry ... time is running out!  Your entry must be postmarked by Saturday, January 15, 2005 to be eligible for the drawing to win the phone call from Hilary Duff and a trip to California!*

Now that you're ready to go, you can print forms in one of two ways....  

Method A - (recommended method)

Method B - (for those without Acrobat Reader)

You can click on the appropriate form link from the list below, where it will open a new web browser window.  Select "print" in your web browser, then close the page to come back here.  Repeat this process again for the next form you need until you have completely printed all the forms or pages you are seeking.  Again, this is not our recommended method and should only be used if you do not have Acrobat Reader or are having difficulty printing the Streamlined Packet, or if you need the three items not included in the .pdf packet.
